How it works

If you are a taxpayer, your signature on this form allows The Shrewsbury and Telford Hospital NHS Trust Charity to claim tax relief on your donation direct from the Inland Revenue.  You must pay an amount of income tax and/or capital gains tax at least equal to the tax that the charity reclaims on your donations in the tax year.

Once you have signed your Gift Aid declaration it is valid for all gifts in the future, and for any gifts you have made to us in the four years prior to signing the declaration.

Please be advised that Gift Aid cannot be claimed on donations made on behalf of a group or a collection.
