Fracture Liaison Service

The Fracture Liaison Service (FLS) is a multidisciplinary service, which aims to identify patients at risk of Osteoporosis. Criteria identified for the service include case finding patients over the age of 50 who have sustained a low impact fall resulting in a fragility fracture, fall/slip or trip from standing height. The primary aim of the service is to prevent subsequent fractures, which improves health outcomes for a rapidly ageing population, improved quality of life, health and well-being and in turn reducing the costs to the NHS.

The FLS service is based at the Royal Shrewsbury Hospital and is a telephone led service for patients who reside under a Shropshire council postcode only; working in partnership with Robert Jones Agnes Hunt Orthopaedic Hospital for referrals on bone health assessments and specialist treatment if required in the metabolic bone unit.

What is osteoporosis?

Osteoporosis is a silent condition where the bones become weaker and break easily, even after a minor bump or fall. Fractures can affect almost any bone, but they are most common in the wrists, hips and spine.

Who is at risk?

  • 1 in 2 women (over 50)
  • During menopause there is a decline in bone strength and may result in a fragility fracture
  • 1 in 5 men
  • The prevalence of osteoporosis increases dramatically with age

Assessments

Patients at risk of osteoporosis are assessed, and if appropriate referred for bone density scans (DXA scan) and/or to commence treatment. Results are discussed with the patient, as well as treatment plans and lifestyle advice, and the patient’s GP will be contacted to commence treatment where required.  The risk of additional falls is addressed with the patient, as are lifestyle and diet, and referrals are made where necessary to the Falls Service/ strength and balance programmes.

Early assessment will lead to improved outcomes in terms of improved quality of life, health and well-being.
